Religious Accommodations

To ensure a supportive and inclusive environment for all SLCC students, students may request religious accommodations. Religious accommodations may include, but are not limited to, rescheduling an exam, rescheduling an in-class presentation, completing a make-up assignment in place for missed class work, a change for an assignment due date, or having an excused absence from the class. Faculty and staff will keep your request confidential and will only be shared to evaluate the process, grant a request, or to process a grievance of a denied request.

Student Religious Accommodations Requests Form

Please submit the form above at least two weeks before the date for which a request is being submitted. You may request more than one accommodation per course. Once the form is submitted, please allow 5 business days for a response. Appeals and Grievances

If your request is denied, you may submit a grievance or appeal to Candida Mumford, Dean of Students, by email at candida.mumford@slcc.edu or by phone at 801-957-5027. The Dean of Students Office will reach out to you for further information or to request an appointment.
