Faculty Resources

Testing Services provides exam proctoring services for any SLCC academic courses. Our Testing Center facilities support both computer-based and paper-and-pencil tests, administered in a quiet and secure setting.

Exam Request Form

Make-Up & Accommodated Exams

We administer make-up exams for students who are unable to test due to illness, a family emergency or military service. This service is available to all courses (including classroom) at our Taylorsville Redwood, Jordan, Herriman and South City locations. Make-up exams are not offered during the week of finals. If your entire class is not taking their final through Testing Services, with the exception of accommodated exams, the exam request is considered a make-up exam and will be offered the following week.

We administer exams for students with accommodations for faculty that do not use our services regularly for testing. If you are using our services for your entire course you do not need to submit a separate request.

Locations

When you submit a make-up exam, we encourage you to select all four Testing Centers (Taylorsville Redwood, Jordan, Herriman and South City) to give students the maximum availability in testing appointments.

How to Submit a Make-up or Accommodated Exam

  • Visit MySLCC and navigate to the 'Testing Services' tile card on your home page. If you do not see the testing tile card, please go to the bottom of the page and click on 'Discover More'. In the search box, type 'testing'. You can pin this tile card to your home page for future submissions.
  • Click on either ‘Submit a Make Up Exam Request’ or ‘Submit an Accommodated Exam Request’ under Testing Services.
  • Once you click the link to RegisterBlast, navigate to ‘Submissions’ and ‘+New’.
  • Fill out all the required information on this form. Some important policies to be aware of:
    • All necessary files must be attached at the time of the request. These files should be in a finalized form and ready to give to students directly without the Testing Center making any alterations (e.g., no answer keys attached, exams with multiple parts must have the two parts attached as separate files).
    • All exams must have a time limit from 30 to 180 minutes.
    • There is a minimum processing time of 24 hours (1 business day) to make the test available for students to schedule. No same-day submissions will be accepted.
    • For paper exams only: completed exams will be scanned and emailed directly to the instructor; as this is the mode of delivery, no scantrons will be given out to students. All answers will be recorded directly on the exam.
    • For both Accommodated and Make-up exams you must edit your Canvas course and make sure your exam is available and/or has the correct amount of time allocated to the student.
    • Make Up exams are intended only for students in the above categories, and there are a limited number of seats available. There is a limit of 5 students per section per make-up test.
    • Make-up testing windows may be open for a maximum of two work weeks.
    • Make-up testing will not be offered during final exam periods.
    • Accommodated exams do not need a separate submission request if the entire course is using Testing Services.
    • Do not include double-time when submitting an Accommodated exam. The system will calculate additional time.

We strongly encourage faculty and students to review our testing policies to prepare for their appointment.

Remote Proctoring

In addition to proctoring within one of our Testing Centers, coordination of arrangements for remote proctoring for online students who live outside of our service area (Salt Lake Valley) is also available. We facilitate communication among the student, instructor, and remote proctor. Staff perform an initial review of the proposed proctor or facility to make sure required standards are met. In questionable cases, staff provide guidance to both the student and instructor regarding appropriate alternatives.
