Concurrent Enrollment

Parent Permission

To participate in concurrent enrollment students and their parents must sign the concurrent enrollment parent permission form. This can be done by clicking on the link at the bottom of the registration page

Below is a copy of the agreement for your reference:

  1. Concurrent Enrollment provides an option for prepared high school students to take courses that earn both high school and college credit.
  2. CE courses are college courses. College course(s) require rigorous academic work. Course content may include controversial cultural, religious political, aesthetic and human sexuality issues. Students must have the maturity to engage with the material in an academically appropriate manner and the organization and time management skills to meet deadlines and complete more rigorous and demanding work than is required in high school. Students must also meet all college prerequisite requirements before enrolling in a CE course.
  3. Students enrolled in online or IVC CE courses may be in classes with college students.
  4. The CE program is open to students in grades 9, 10, 11, and 12. Students must be enrolled in a Utah public high school and be counted in the average daily membership before and through enrollment in a CE course. Students who have received a diploma, whose class has graduated from high school, or who have participated in graduation exercises are not eligible to participate in the CE program. Home-schooled students are eligible to participate in CE courses with local LEA permission. Home-schooled students are subject to the same obligations as any other student in the CE program.
  5. Students may attempt up to 30 CE credits per year and may not repeat a CE course.
  6. CE course registrations and grades are recorded on permanent high school and college transcripts. Transcripts will be used to evaluate eligibility for scholarships and financial aid and for admission to certain academic programs.
  7. Withdrawing from or failing a CE course may impact a student's ability to receive financial aid in the future as well as a student's ability to enroll in additional CE courses.
  8. SUBMISSION OF THIS FORM DOES NOT ENROLL THE STUDENT IN CE COURSE(S). The student is responsible for logging into the college website to complete the course enrollment process and pay CE course tuition.
  9. If a student withdraws from a course at the high school, s/he must also withdraw from the course at the college. Failure to do so may result in a failing grade on the college transcript.
  10. CE credit earned shall be transferable from one Utah public institute to another. Students should seek advice from a higher education academic advisor to make course choices that meet their educational goals and transfer as equivalent credit.
  11. CE students must abide by the college Code of Student Rights and Responsibilities as well as the high school Student Code of Conduct.
  12. Students must contact the college CE administrator to request any accommodations that may be required under the Americans with Disabilities Act and Section 504 of the Rehabilitation Act.

I certify that all information on this CE application is complete and true to the best of my knowledge. I understand that knowingly falsifying or withholding information may result in a loss of CE credit or removal from the CE program.

I understand that the CE participation fees are not refundable.

This electronic signature consists simply of your name, typed by you on your keyboard. The signature is your confirmation that the CE application you have filled out is your own work and the information is factually true.
