Patient Portal
Click the link below to login using the SLCC Microsoft Authentication process. See below for information about logging into your account.
Once logged in, you will have access to the following:
- Online patient forms – Please complete forms prior to your appointments!
- Lab results from your Providers
- Online scheduling for Rapid STI/STD testing – Visit “Appointments” to get started.
- The ability to message your provider
- The ability to cancel upcoming appointments (if done with +24 hours' notice)
When accessing the Patient Portal, you should be able to view your upcoming appointment(s) as shown below. If you don’t see an upcoming appointment on your account, call the Center for Health & Counseling to schedule an appointment. If you have paperwork to complete prior to your appointment, you will see how many forms that need to be completed in green as shown in the example below.

Need help logging into your account?
To log into the Patient Portal you will need to know your SLCC email and password as well as have access to the Microsoft Authenticator app or click on “verify using text message” which will send a verification code to your cell phone to log in.
If you are having issues with the login process, you will need to call IT – 801-957-5555.
