Step 1: Student Consent for Release of Records
Students must download the Student Consent for Release of Records form, print, physically sign the document, and scan or send a picture of the signed form along with a current (unexpired) official picture ID when submitting this form in person and online. Send documents to pdregistration@slcc.edu.
Step 2: MIDAS Credit
MIDAS Education is a database provided by Utah State Board of Education for Utah educators to track and provide transcripts of their professional development credits. MIDAS credits are recognized by most districts in the state as evidence for a lane change.
Teachers who are seeking Health & PE Endorsements credits will automatically receive MIDAS Credits.

These two endorsements are designed for K-12 teachers and were created in partnership with Utah State Board of Education (USBE). These not-for-credit courses are ideal for current teachers who are looking to further their careers. These endorsements can be completed in two semesters and translated into 2 credits per class through MIDAS-USBE.

PE : This program is designed for K-12 educators seeking a PE endorsement from the Utah State Board of Education (USBE). Courses include Methods of Teaching Physical Education and Activities, Methods of Teaching PE Skills/Individual Lifetime Activities, Methods of Teaching Fitness for Life, and Adapted PE. As a result of this training, students will be able to develop and implement various physical education lesson plans aligned to the Utah Core Standards and adapted for students with variable physical and cognitive abilities, monitor and evaluate student motor skills progression and exercise physiology in lifetime activities and fitness-for-life activities, develop and evaluate safe and effective personal fitness plans and how they impact mental, social, physical, and emotional well-being, and integrate adapted physical education skills, movement, and sports that allow students with disabilities to participate with their peers.

Health: This program is designed for K-12 educators seeking a health endorsement from USBE. Courses include Methods of Teaching Health; Mental Health and Substance Abuse Prevention; Nutrition, Safety, and Disease Prevention; and Human Development/Human Sexuality. As a result of this training students will be able to develop and implement lesson plans aligned with Core Standards for Human Sexuality, Mental Health and Wellness, Nutrition, Safety, and Disease Prevention, and Substance Abuse Prevention; apply trauma informed teaching practice in lesson development and instruction; examine the effects of healthy relationships on all aspects of health and wellness.

The PE and Health Endorsement program is eligible for STIT funding to cover 50% of tuition. Utah residency is required. To see if you qualify for STIT Funding and to complete the application:
