When you get your flight itinerary confirmation, please forward it to accountspayabletravel@slcc.edu.

To request per diem cards for student travelers do the following after you have completed your Group T Number:
1) Enter a Payment Request to vendor USBank Prepaid in SLCCBuy for the total amount needed. This will fund your card account.
2) Complete the Per Diem Card Request Form and email it to Lisa Bradshaw lisa.bradshaw@slcc.edu.
